Targeted Case Manager – Children (TCM)
Full-Time or Part-Time$18 – $24/hour depending on experience and credentialsMonday–Friday; some community travel required; school-year schedule flexibility
About This Role
Provide Targeted Case Management (TCM) services to children enrolled in MaineCare. Work with children and their families to coordinate care across home, school, and community settings.
Responsibilities
- Develop and implement individualized care plans with children and families
- Link families with educational, medical, mental health, and community resources
- Coordinate services across providers and school systems
- Provide advocacy and support to families navigating complex systems
- Maintain timely documentation in compliance with MaineCare standards
- Participate in team meetings and clinical supervision
Requirements
- Bachelor's degree in social work, psychology, education, or related field
- Experience working with children and families in a community or clinical setting preferred
- Knowledge of Maine child welfare and MaineCare systems a plus
- Strong communication, organizational, and documentation skills
- Valid Maine driver license and reliable transportation
