BHH Case Manager
Full-Time or Part-Time$18 – $24/hour depending on experience and credentialsMonday–Friday, flexible hours; some community-based travel required
About This Role
Provide Behavioral Health Home (BHH) case management services to adults and/or children enrolled in MaineCare. Help clients navigate community resources, build self-advocacy skills, and work toward independence.
Responsibilities
- Conduct comprehensive assessments and develop individualized care plans
- Link clients with community resources (housing, benefits, medical, social services)
- Coordinate care across providers and systems
- Provide psychoeducation and skill-building support to clients and families
- Complete required documentation and billing in a timely manner
- Participate in multidisciplinary team meetings
Requirements
- Bachelor's degree in social work, psychology, human services, or related field
- Experience in behavioral health, case management, or social services preferred
- Familiarity with MaineCare systems and community resources in Maine a plus
- Strong organizational, communication, and documentation skills
- Valid Maine driver license and reliable transportation
